Puppy Application -
If you are interested in obtaining a puppy from Summit Farms please fill out the following questionnaire. You may need to copy and paste it into word, note pad, etc. and then email it.
Our puppies are very special to us and therefore we feel it is our responsibility to make sure they get the best possible forever home we can
find for them. This questionnaire will help us learn about you, your family, your home, and your lifestyle. That in turn will help us to match the right puppy to you and your specific family and your lifestyle. We will not make a final decision on placing any of our puppies until after eight weeks and we have a evaluation on each puppies temperament and structure.
Filling out this application does not guarantee a puppy will be placed with you in your home. We do not ship puppies.
